It's not the sort of opponent that these valorous knights expected to fight — not an enemy army, not even a huge monster, but a horde of tiny gluttonous beasts, able to pick a soldier to the bone, armor and all. Kill the queen, and the swarm will scatter. Every minute spent on this quest will be dearly bought by the lives of those busily distracting the bulk of the vescavors. Somewhere in a cave nearby, there are valuables that the beasts will destroy if they're not quickly retrieved. It comes down to a choice: life or treasure.

Walkthrough[ | ]
- Upon entering Leper's Smile location you will get a Book Event (Leper's Smile).
- You get to choose a commander for suicide squad: Irabeth, Nurah, Regill, Lann, Sosiel.

- You lose extra 30xp per point of [GS_ArmyLosses] of cheapest army unit.
- Choosing Sosiel and holding ground is obviously the best choice.
- After the Book Event you get to choose your team composition for the location.
Get to the swarm queen[ | ]
- On the location itself you will first meet Anevia, if your deity is Gozreh or Green Faith then you can buff your party for 1 hour with Gozreh's Blessing or Green Faith's Blessing respectively.
- Some of the loot on the map will trigger the addendum lines, which will rise your [GS_TimeLosses] key flag.
- You will lose Morale -5 as well as army units of 10 Experience and 5% for each rise of the flag.
- You can discover which of them do by save-load, the rest of the loot is free for the taking.
- In the middle of the map you will meet a crevice with a Lost Traveler.
- In order to save him you need to pass Perception DC 17check(+17xp) and Athletics DC 20check(+24xp) in order to bring him up alive.
- Saving him requires a (Good) alignment choice and a way to apply Cure Serious Wounds, Cure Critical Wounds or Heal. You will get to meet him in Act 4 and he will give you Ashmaker as a reward.
- Both saving him or finishing him off will result in rising of [GS_TimeLosses] key flag with it's consequences. Leaving him be will have no effect (although he will die that way).
Discuss the finds with the companions[ | ]
- After defeating Vescavor Queen you get to discuss your results.
- If you still have Perfumed Handkerchief on you, then with Anevia you get to pass hidden Perception DC 26check(+75xp) to mention the handkerchief, or you can react to it without check.
- If you dropped the Perfumed Handkerchief you will miss out on etude that is useful for later events in Drezen. It's better to keep it on you.
- With (Evil) choice you can order Anevia to gather some of the Vescavor Queen's slime which you could use during siege of Drezen and is required for unlocking Mythic Swarm.
- After that Nurah and chosen suicide squad commander comes and you get to discuss your losses with (Good/Lawful/Chaotic/Evil/None) alignment choice.
- If [GS_TimeLosses] is 2 or more then you also lose army units of 10 Experience and 10%.
- If squad commander was one of your companions then you will get an opportunity to swap your team at the end of dialogue.
Final Army Loss[ | ]
The army will suffer a loss of percentage from total number as well as extra amount based on units Experience value.

Side Activities[ | ]
- Leper's Smile also have the cave section where you can find Wand of Zacharius which will periodically reanimate the wights. Click on it quickly after defeating them.
- You can break it with (Good) action and get Broken Wand of Zacharius.
- You can unleash it's powers with (Evil) action and destroy the Wights (you still gain kill experience).
- If your deity is Pharasma you will get Pharasma's Curse for 1 hour.
- You can pick it up with normal action, but you will have to kill the Wights.
- The Wand of Zacharius is required to both unlock Mythic Lich and additional sub-location of Lost Chapel where a number of unique loot can be found.
- Also at the same place you can find A Piece of Terendelev's Claw which is needed for The Dragon's Fate.
